You’re doing everything you’re told to, yet the results don’t match the effort. The reassuring truth is that cavity risk isn’t decided by oral hygiene alone. A whole set of factors — some within your control, some simply part of how you’re built — influences how vulnerable your teeth are to decay. Once you understand them, the picture starts to make a lot more sense. ## What actually causes a cavity A cavity is the end result of a slow process, not a sudden event. Specific strains of bacteria in your mouth feed on the sugars and starches you eat and produce acid as a byproduct. That acid gradually draws minerals out of enamel, the hard outer layer of the tooth, and over time that weakened spot can break down into a hole. The key word is gradual. Your mouth swings between acidic phases, after eating, and recovery phases in between, when saliva restores balance. Decay takes hold when the acidic phases consistently outweigh the recovery. That balance is exactly what varies so much from one person to the next. ## The bacteria you carry isn’t the same as everyone else’s Not all mouths host the same microbial mix. Some people naturally carry higher levels of the bacteria most associated with decay, which means the same diet and the same brushing routine can lead to very different outcomes. This is partly why decay can seem to run in families or cluster in certain people. It isn’t a verdict on your effort — it’s a difference in the starting conditions. It also explains why two people can share almost identical habits and end up with very different check-up results. ## Saliva: your mouth’s quiet defence system Saliva does far more than keep your mouth comfortable. It rinses away food particles, neutralises acid after meals, and delivers the minerals that help rebuild enamel between eating occasions. A healthy saliva flow is one of the strongest natural protections you have. When saliva is reduced, that protection drops away. Dry mouth can stem from certain medications, mouth breathing, some health conditions, or simply individual biology. People who run dry often see more decay despite good habits, because their enamel spends longer exposed to acid without relief. ## Enamel isn’t identical from person to person Enamel thickness and quality vary. Some people’s enamel is naturally thinner or more porous, which makes it easier for acid to penetrate. This can be genetic or shaped by influences during the years teeth were forming. Thinner enamel doesn’t mean cavities are inevitable. It does mean the margin for error is smaller, so the everyday choices around diet and care carry a little more weight than they might for someone with robust enamel. ## Why how often you eat matters more than what you eat It’s tempting to focus on banning particular foods, but frequency usually matters more than any single item. Each time you eat or drink something sugary or acidic, your mouth tips into an acidic state for a stretch afterward. Constant grazing keeps it there. Three meals with little snacking in between gives enamel long, uninterrupted recovery windows. Six small eating occasions spread across the day keep your teeth under near-continuous acid exposure, even if the total amount of food is the same. Sipping sweet or acidic drinks slowly has the same effect. ## When to talk to your dentist If cavities keep appearing despite your best efforts, that pattern itself is worth a conversation. Recurring decay is information — it points to a specific driver that can be identified, whether that’s dryness, diet timing, enamel quality, or something else. A tailored approach might mean more frequent check-ups, a closer look at your daily routine, or guidance suited to your particular risk profile, rather than generic advice that assumes everyone starts from the same place.
