Palatal expansion (also called maxillary expansion) is a well-established orthodontic technique used to gently widen the upper jaw. At TMJ Therapy Centre, we use expansion not just to straighten teeth, but as part of an airway-focused, TMJ-informed treatment philosophy that supports breathing, sleep, and jaw function for life.
A narrow upper jaw is one of the most under-recognised drivers of crowded teeth, mouth breathing, snoring, poor sleep, and even TMJ pain later in life. Expansion creates physical space — for the teeth, the tongue, and the airway — and is most effective when started early, though carefully selected adults can also benefit from modern expansion approaches.
Palatal expansion uses a custom dental appliance — fixed or removable — to gradually widen the upper jaw (maxilla) along the midline suture. In children and adolescents, the suture has not yet fully fused, so gentle, sustained pressure allows the two halves of the upper jaw to separate slightly and rebuild with new bone in the widened position.
In adults, the suture is generally fused, but modern techniques such as MARPE (Mini-Implant Assisted Rapid Palatal Expansion) and surgically assisted expansion can achieve genuine skeletal widening in carefully selected cases — not just tipping the teeth outward.
The growth window is the ideal time to expand. Treatment is faster, gentler, and provides the strongest long-term airway and jaw benefits.

We evaluate jaw width, palatal vault, tongue posture, breathing patterns, and bite relationship — alongside any TMJ or sleep concerns. Imaging may include intraoral scans and, where indicated, a cone-beam CT to assess the suture and airway.
Depending on age and need, we use a removable expansion plate, a fixed rapid palatal expander (RPE), Myobrace-style functional appliance, or a MARPE expander for adults. The appliance is fitted precisely and you receive clear activation and care instructions.
A small key is turned at home — typically once a day for rapid expansion, or less often for slower protocols. The movement is gentle; most patients feel only mild pressure that settles within minutes.
Once the target width is reached, the expander stays in place for several months to allow new bone to fill in stably. We then transition to retention and, if needed, the next phase of orthodontic, myofunctional, or TMJ care.
Airway & nasal breathing. The roof of the mouth is the floor of the nose. Widening a narrow upper jaw opens the nasal floor, improving airflow and supporting nasal breathing — a foundation for better sleep, focus, and jaw posture.
Sleep & snoring. A wider palate gives the tongue room to rest on the roof of the mouth rather than collapsing back into the airway, which can reduce snoring and contribute to managing sleep-disordered breathing.
TMJ & bite. A narrow maxilla often forces the lower jaw into a compromised posture, contributing to clicking, muscle tension, and TMJ dysfunction. Expansion lets the lower jaw seat into a more comfortable, balanced position.
Myofunctional support. Expansion works best alongside orofacial myology — retraining the tongue, lips, and breathing patterns so the new width is stable for life.
